Inadequate monitoring/ enforcement PRIMARY CAUSES MANAGEMENT WORKERS DIFFERING PERSPECTIVES: MANAGEMENT COMPARED WITH WORKERS Management and workers were asked to vote on the level of importance of each primary causal factor for inadequate lockout/tag out practices. While there was variation, the cohort generally scored closely on most topics. This suggests there is a shared understanding of many factors influencing behaviours relating to lockout/tag out. There were some noteworthy differences. For example, management tended to give more weight to gaps in leadership commitment and perception of risk. Worker representatives identified lack of procedures and training as a significant gap.
